Infections: Rubella status, Syphilis Screen (RPR), Hep B surface antigen, HIV counseling and testing, Chlamydia, Gonorrhea (GCC), TB if high risk.
6-8 Weeks:
History and Physical: BP, Wt, BMIFetal Heart Tones.
Labs: Ab Screen if not done at initial, Genetic screen if not done at initial
Urine Dipstick
Ultrasound (Crown-Rump Length)
10-12 Weeks:
Chorionic Villus Sampling (if high risk)16-18 Weeks:
History and Physical: BP, Wt, Fundal Ht, Fetal Heart TonesLab: Urine Dipstick, US (BPD, HC, FL, AC)
Amniocentesis if high risk
Pentascreen
26-28 Wks:
History and Physical: BP and Wt, Fundal Ht, Fetal Position and FHTsLab: H/H, Gestational DM screen, Ab Screen and Rhogam IF RH NEG.
Urine: Dipstick, UA and Urine Cx
32 Weeks:
History and Physical: BP, Wt, Fundal Ht, Fetal Position and FHTsUrine Dipstick
36 Weeks:
History and Physical: BP and Wt, Fundal Ht, Fetal Position and FHTsLabs: CBC
